In situations where simultaneous breast reconstruction is not possible, the breast reconstruction surgery should be performed after the completion of chemotherapy and at least 8-12 months after the end of radiotherapy. Ultimately, the timing of the reconstructive surgery depends on the recovery of normal bone marrow function and the resolution of radiation-induced changes such as inflammatory reactions, vascular changes, or fibrosis.
What does the choice of breast reconstruction method depend on?
The choice of method depends, among other things, on: the appearance and size of the other breast, indications or contraindications for using autologous tissue or silicone implants, the patient's preferences
How is the procedure using artificial materials performed?
Procedures using implanted materials are most often performed in women who: • have not undergone radiotherapy, • have an adequate volume of tissues, i.e., when the pectoralis major muscle is preserved and the appropriate thickness of the skin-fat folds is maintained. The first stage of reconstruction is the implantation of the so-called expander, which is gradually enlarged to achieve tissue stretching. The expander is placed in a specially prepared pocket under the pectoralis major muscle. Over several months, gradual expansion of the expander takes place through successive filling with saline. After achieving the appropriate tissue expansion, during the next surgery the expander is removed and replaced with the final silicone implant. The choice of implant depends on the size and shape of the reconstructed breast and requires precise measurements. Currently, various types of round and anatomical implants are available. They differ in surface texture—smooth or textured—and in the degree of silicone density. Textured surfaces have different pore sizes, allowing varying degrees of integration with tissues. It often happens that the other breast is particularly large or significantly differs from the reconstructed breast; in such cases, reduction or symmetrization surgery can be considered. This allows achieving symmetry between the reconstructed and the patient's own breast.
What does the nipple reconstruction procedure look like?
The final element of reconstructive treatment is the restoration of the nipple and areola, which can be performed after completing the shaping of the reconstructed breast and symmetrization of both breasts. Various methods are used to restore the nipple and areola: tattooing – tattooing the areola and nipple in the appropriate place on the reconstructed breast; reconstruction of the nipple and areola from the patient's own tissues – skin graft or transplantation of a nipple fragment from the other breast; combination of both aforementioned methods.
What do breast reconstruction procedures using the patient's own tissues look like?
One of the most popular breast reconstruction methods for women treated with radiotherapy or those not irradiated but with a large loss of soft tissues is the procedure of transplanting a skin-muscle flap from the back with simultaneous use of an expander, followed by its replacement with a silicone implant. This involves minimal deformation of the donor site – a scar remains visible on the back. The main advantage of this method is the increased volume of soft tissues covering the prosthesis, which provides a more natural appearance of the reconstructed breast and allows better matching to the healthy breast. The most popular technique using only the patient's own tissues for breast reconstruction is the transfer of a flap from the rectus abdominis muscle.

Sensation may be partially limited, but often gradually improves over time. The degree of sensory recovery is individual.
